Add to Medical History (All Categories)

To add Medical History, go to the patient health record click on the PMFSHx button. Choose between the data essential to the proper diagnosis and development of the most appropriate plan of care.

• Medical Hx (Medical, Mental Health, Reproductive, Hospitalizations, Falls) 

• Family Hx 

• Social Hx (Personal, Tobacco, Alcohol, Substance Use) 

• Surgical Hx (Surgeries, Implants, Procedures) 

  • Social Determinants of Health/SDOH (Stress, Financial, Physical, Access to Home, Pets, Appt. Logistics, Shopping/Meals, Spiritual, Folk Practice) 

Once identified click on the Add button, enter all the details then click on the Save and Done button.
