Use and explain the Health Maintenance feature

The Health Maintenance/CQM tab of the EHR helps you track your patients’ health maintenance exams and screenings, such as mammograms and colorectal cancer screenings. Tracking these measures can help you focus on disease prevention and help you ensure your patients receive the right care at the right time. In addition, this data can help you demonstrate meaningful use of Clinical Quality Measures (CQMs) for reporting in quality analysis programs. 

The Advance Medical Directives section, accessed by the tab at the top of the screen, is an interactive tool developed to help you assist patients with the creation of personalized AMDs.


Each record includes: 

 

• Measure: The specific exam or screening 

• Constr: The recommended age constraint 

• Freq: The recommended measure frequency 

• Status: The status of the measure (Performed, Not Performed, Hospice, etc.) 

• Date: The date of the measure 

• Results: A brief summary of the results
